Your teen is 16 or 17 and wants to play in the adult league? Fill out and sign the form, then hand it in before their first game.

What the form contains

You authorize your teen to play in the adult league, and you acknowledge that the sport carries a risk of hits and injuries.

The document also includes a release of liability in favour of the league.

The completed, signed form is handed in before the first game: in person to the administration, or by email to dekhockeybeauchateau@gmail.com.

At 16 and 17, your teen must wear a full face protector, in addition to the mandatory equipment.
